  2. Proof of Address
    A recent utility bill (e.g., telephone, electricity) or a bank statement dated within the last three months, showing your full name and address.

  3. Proof of Payment
    For new credit or debit cards used for deposits:

    • Front: A color copy displaying the last four digits, expiry date, and cardholder name.
    • Back: A color copy showing the last four digits and the cardholder’s signature.

  4. Deposit Declaration Form
